Cerebral tissue oxygenation index and SIDS.

چکیده
In this issue of Pediatrics, Fyfe et al present convincing evidence that cerebral tissue oxygenation index (TOI) is lower in habitually supinesleeping preterm infants placed prone rather than supine, and that the cerebral TOI for preterm infants placed prone is lower than the TOI for term infants placed prone. Increased rates of sudden infant death syndrome (SIDS) among all infants sleeping prone, and even higher rates among preterm infants placed prone, provide the epidemiologic rationale for their physiologic studies.

متن کاملCerebral oxygenation in preterm infants.
BACKGROUND AND OBJECTIVE Prone sleeping is a major risk factor for sudden infant death syndrome (SIDS) and preterm infants are at significantly increased risk. In term infants, prone sleeping is associated with reduced mean arterial pressure (MAP) and cerebral tissue oxygenation index (TOI). However, little is known about the effects of sleeping position on TOI and MAP in preterm infants. We ai...
متن کاملContinuous assessment of cerebral autoregulation with near-infrared spectroscopy in adults after subarachnoid hemorrhage.
BACKGROUND AND PURPOSE In patients with subarachnoid hemorrhage, the assessment of cerebral autoregulation aids in prognosis as well as detection of vasospasm. Mx is a validated index of cerebral autoregulation based on measures of cerebral perfusion pressure and mean flow velocity on transcranial Doppler but is impractical for longer-term monitoring. Near-infrared spectroscopy is noninvasive a...
متن کاملImpaired autoregulation in preterm infants identified by using spatially resolved spectroscopy.
OBJECTIVE The absence of cerebral autoregulation in preterm infants has been associated with adverse outcome, but its bedside assessment in the immature brain is problematic. We used spatially resolved spectroscopy to continuously measure cerebral oxygen saturation (expressed as a tissue-oxygenation index) and used the correlation of tissue-oxygenation index with spontaneous fluctuations in mea...
متن کاملEffect of closed minimized cardiopulmonary bypass on cerebral tissue oxygenation and microembolization.
OBJECTIVE Coronary artery bypass grafting with cardiopulmonary bypass carries a risk for neurologic complications because of cerebral hypoperfusion and microembolization. The basic goals of a novel closed minimized extracorporeal circulation are to prevent excessive hemodilution and to avoid blood-air interface. The aim of this prospective randomized study was to determine the effect of using t...
